Media Communication Center Equipment Request Form

Reservations are made on a first-come, first-served basis, with priority give in this order: CMCO Classes, CMCO Course Assignments, Faculty, Student Internship Projects, Non-CMCO Projects
All reservations are to be made 24 hours before the equipment is required. Requests for Monday should be made no later than Friday.
You are responsible for pickup and return of equipment.
Check-out is for one day during the week or Friday through Monday on most weekends. See calendar for heavy use weekends.
Special requestes must be made in writing and approved by the professor in writing.
Equipment check in/out times are:
Monday - Friday Check in 7:30-8:30 a.m. and Check out 4:30-6:00 p.m.
