Immunizations/CPR/Background Check

Health Certification Requirements

  • Proof of current immunications for polio, diphtheria, tetanus, second MMR, and hepatitis B vaccination
  • Titer for varicella (chickenpox) is required only if you do not know when you had the disease. If you have had chickenpox, include year of disease on health record.
  • Negative Mantoux or PPD (TB) needs to be current through the school year. (Students must provide a copy of an updated test for their file each year.)
  • Positive Mantoux or PPD - provide documentation of a negative chest x-ray with application. If chest x-ray is positive, the student needs to see their health care provider. Admission to the nursing program may occur only after the department receives verification of a negative sputum culture.
  • The Health Certification form is a part of the nursing department application.

Healthcare Provider CPR Certification

  • Must be current
  • Obtained from American Heart Association (recommended) or Red Cross
  • All students are required to provide a copy of current Healthcare Provider CPR certification with application.

Criminal Background Check

  • Required after admittance into the nursing program
  • Cost: $25.00 (credit card only)
  • Background checks must be on file, in the nursing department office, prior to first clinical rotations.

Mandatory Drug Testing

  • Required after admittance into the nursing program
  • Cost: $38.00
  • Results of drug test must be on file, in the nursing department office, prior to first clinical rotations.
