Immunizations/CPR/Background Check
Health Certification Requirements
- Negative Mantoux or PPD (TB) MUST be kept current throughout the program. Student must have an initial 2-step PPD documented in their student file and then an annual 1-step PPD test thereafter kept in their file. TB skin tests are to be taken annually for as long as the student is in the program. If a student has a Positive Mantoux or PPD, they will need documentation of a negative chest x-ray to be submitted with application. If chest x-ray is positive, student needs to see their health care provider. Admission to the nursing program may occur only after the department receives verification of a negative sputum culture.
- Proof of current immunizations:
- Polio, Diphtheria, Tetanus (tetanus only within past 10 years)
- Two (2) MMR, or titer drawn if you have not had two MMR vaccinations
- Series of three (3) polio PLUS one (1) booster
- Series of three (3) Hepatitis B, or titer
- Proof of either two (2) Varicella (Chicken Pox) vaccinations or one (1) titer for Varicella. Having had chicken pox is not sufficient documentation.
- The Health Certification form and all documentation (copies of medical records and/or immunization cards) is a part of the nursing department application.
Healthcare Provider CPR Certification (American Heart Association)
- Obtained from American Heart Association only.
- Must be current and will not expire until the summer after your first semester in the program, at the earliest.
- All students are required to provide a copy of current Healthcare Provider CPR card with application.
- Certification must be kept current throughout your time in the nursing program.
Criminal Background Check
- Required after admittance into the nursing program
- Cost: $25.00 (credit card only)
- Only background check arranged by the nursing department is accepted.
- Background checks must be on file, in the nursing department office, by the start of your first semester in the program.
Mandatory Drug Testing
- Required after admittance into the nursing program
- Cost: $38.00
- Only drug test scheduled by the nursing department is accepted.
- Results of drug test must be on file, in the nursing department office, by the start of your first semester in the program.
