Sign up Instructions and Forms for May 2012 Trips

• Course Descriptions and Registration Information

• Medical Release Form (due December 2, 2011) The Medical Release Form should be printed, signed and submitted to the Health and Counseling Center by December 2, 2011.

• Passport Instructions and Application Form (Passport is due January 13, 2012)

• Deposit form (due January 13, 2012) The Deposit form should be downloaded and completed in WORD, printed, signed and turned in with you deposit to Student Financial Office.

• ISIC Card The ISIC form should be completed in WORD, printed, signed and submitted to the Juniors Abroad office (Woodmar 204) Not required for everyone.

• Release of Information Form  (due January 13, 2012) This form should be downloaded, printed, filled out, signed, and turned into  the Juniors Abroad office (Woodmar 204)
