Disability Services Office
Requesting Services
The Disability Services Office (DSO) provides services for students with disabilities.Following is an overview of the process for arranging accommodations.
- Complete a Disability Services Request Form. (The form requires Adobe Acrobat Reader)
- Provide current documentation of your disability.
- Send the completed Disability Services Request Form and documentation to rmuthiah@georgefox.edu or mail them to the address below.
- Meet with the DSO to discuss possible accommodations.
- The DSO will prepare an accommodation letter for you to take to your instructors.
- Give a copy of the accommodation letter to your instructors.
- Talk over anything that needs to be clarified with your instructor.
- Communicate with the DSO if there are any questions or difficulties along the way.
