IMS Equipment Request

IMS Equipment Request Form


Reccuring requests can be made with one submission.
  • Same day and time each week. For a Tuesday and Thursday class, enter T TH with start and end time. A request for every Tuesday and Thursday will be entered for the entire semester.
  • Same time and varied dates. Enter the start and end times in the appropriate field. List dates in the additional comments field. A request will be entered for each day indicated.

Need two slide projectors or camcorders? Enter the item in the equipment 1 and 2 field to receive 2, etc, or indicate in the additional comments the number needed.

Request for LESS THAN 24 HOURS (Requests for Sunday or Monday need to be received by 5pm on Thursday to be considered more than 24 hours.):

These requests are to go directly to Piper or Kandie. If they are not available, contact the person covering Tier 2.
