IMS Production Request Form

Students - this form is a way for you to enter information about an IMS Production Request. Enter the quantity of the item requested in the appropriate line. If you don't see what you need, put it in special instructions. We can digitize video for DVD or the web, duplicate media and convert media to CD, DVD or VHS.

Duplication to customer's blank DVD or CD (Free)
VHS Video tape duplication - 60 or 120 minute tape and duplication ($2 each)
Duplication to customer's blank VHS tape ($1 each)
DVD or CD purchase ($1 each)
DVD or CD duplication with printed label ($1.70 each)
MiniDV tape purchase ($6 each)

For most duplication requests we can usually complete the duplication in 24 hours. Digitizing video the turn around time is about a week.
