MPS Equipment Request Form

  • Reservations are made on a first come, first served basis.
  • **NEW**All reservations are to be made by 1pm the day before the equipment is required. If your request is for the next day and it is after 1pm please call the Service Desk at X2569. Requests for Saturday, Sunday or Monday need to be made by 1pm on Friday!
  • You are encouraged to make recuring reservations for the entire semester at the beginning of the semester.
  • Equipment will be set up 10 minutes prior to the requested time. Requester should be present to insure proper setup of equipment.
  • If a customer pick up is requested, the customer is responsible for pickup and return of equipment.
  • You will receive confirmation of your reservation by email within 24 hours of sending your online request.
  • Student organizations may reserve equipment through their faculty/staff sponsor. Please have the faculty/staff sponsor submit an MPS Equipment Authorization Form.
  • Students may reserve equipment for academic use through their instructor or academic department. Please have the instructor or academic department submit an MPS Equipment Authorization Form.

Reoccuring requests can be made with one submission.

  • Same day and time each week. Enter the day of the week, instead of the date, for semester long reqests.  For a Tuesday and Thursday class, enter T TH with start and end time of the class as show in mygfu. A request for every Tuesday and Thursday will be entered for the entire semester, excluding holidays and finals week.
  • Same time and varied dates. Enter the start and end times in the appropriate field. The times should be the actual start and end time of the event or class.  We will back up the time to allow for the setup of the equipment.  List dates in the additional comments field. A request will be entered for each day indicated.

Need two data projectors or camcorders? Enter the item in the equipment 1 and 2 field to receive 2, etc, or indicate in the additional comments the number needed.

Please provide a layout of the room if the event or activity will be in a non-classroom (e.g. Cap and Gown, Bauman Greeen Room or another room with various room arrangements).
