Placement Questionnaire
In order for the Custodial office to schedule your work hours and cleaning area you will need to fill out a Placement Questionnaire. Please download and complete the Placement Questionnaire below. You should be able to save the completed PDF and send it via email as an attachment to Laurie Lorenzen at llorenze@georgefox.edu.
If you are unable to save the PDF with your information then print out the completed form and, if off campus, mail to:
