Disability Services Office

Requesting Services

The Disability Services Office (DSO) provides services for students with disabilities.

Following is an overview of the process for arranging accommodations.
  • Complete a Disability Services Request Form. (The form requires Adobe Acrobat Reader)
  • Provide current documentation of your disability.

  • Send the completed Disability Services Request Form and documentation to rmuthiah@georgefox.edu or mail them to the address below.

  • Meet with the DSO to discuss possible accommodations.

  • The DSO will prepare an accommodation letter for you to take to your instructors.

  • Give a copy of the accommodation letter to your instructors.

  • Talk over anything that needs to be clarified with your instructor.

  • Communicate with the DSO if there are any questions or difficulties along the way.
