Processing time begins upon receipt of request and payment.
Official transcripts will not be released until all financial obligations to the university have been met.
Transcript requests will be held for a maximum of one year.
Failure to fill out the form completely may delay processing.
Please include the following information:
Full Name as it appears on your academic record. Please also indicate any former names with might be included in our university records.
Social Security Number and/or GFU ID#.
Dates of Attendance. Please include whether you attended in an Undergraduate or Graduate program, and whether or not you completed your degree.
Address of Transcript Recipient. Please complete the mailing label if using the request form. For multiple addresses, please fill out additional request forms. In addition, please complete the duplicate box so we can maintain a record for you.
Return Address, Daytime Phone Number, and/or E-mail Address to contact you if necessary.
Your Signature and the Date. Unsigned requests will not be processed.
Your Payment. Payment is expected with the request. We will not process your request without your payment.
