Payment Selection Form

In order to register and begin classes, students must fill out and turn in a Payment Selection Form.

PLUS Loan Request Form

A PLUS Loan is taken out by the parent(s) to help pay the cost of school for their undergraduate son or daughter, or by a graduate student to help cover their own costs.

Verification Worksheets

Each year the U.S. Department of Education radomly selects approximately 30 percent of those who complete the FAFSA for a process called Verification. This does not mean you did anything wrong; it is just a means of ensuring that the FAFSA was filled out properly. We also select some students for verification that have not been federally selected.

When you apply for federal student aid, your answers to certain questions will determine whether you're considered dependent on your parents or independent. If you are considered dependent on your parents, you must report their income and assets as well as your own. If you are independent, you must report only your own income and assets (and those of your spouse, if you're married).

Students are classified as dependent or independent because federal student aid programs are based on the idea that students (and their parents or spouse, if applicable) have the primary responsibility for paying for their postsecondary education. Students who have access to parental support (dependent students) should not receive need-based federal funds at the expense of students who do not have such access (independent students).

You are an independent student if at least one of the following applies to you:

  • you were born before Jan. 1, 1985 (08-09 AY)
  • you are married
  • you are enrolled in a graduate or professional educational program (beyond a bachelor's degree)
  • you have legal dependents other than a spouse (that you provide more than half of the support for)
  • you are an orphan or ward of the court (or were a ward of the court until age 18)
  • you are a veteran of the U.S. Armed Forces ("veteran" includes a student who attended a U.S. military academy who was released under a condition other than dishonorable) or you are currently on active duty for purposes other than training. If you claim to be an independent student, you may be asked to submit proof before you can receive any federal student aid

Special Circumstances Form
If your family has experienced highly unusual events that have had an impact upon your financial situation, you may fill out a Special Circumstances form. A committee will review the information and make a decision. Unusual circumstances include loss of employment or unexpected medical bills. More common circumstances such as paying for dental braces are not considered unusual in nature.

Free Application For Federal Student Aid (FAFSA)
We recommend that you file your FAFSA form on the Web. The process is easy and saves your information from year to year, so it is simple to update. Click here to go to the electronic filing site.

Church-University Matching Form
George Fox University recognizes the importance of Christian higher education in preparing students to serve Christ and the church. The university has established the Church/University Fellowship Award Program to partner with churches that offer scholarship programs to help financially support their students who are attending George Fox University. Approval is required and restrictions apply. Program requirements, matching policies, timelines and program application can be found in the Church/University Fellowship Program Application. We have 2 separate forms, one for traditional undergraduates and one for graduate students in the George Fox University Seminary. The deadline for this application is August 1st (postmarked).

FERPA Release Form
The FERPA form is completed by a student in order to give Student Financial Services permission to speak with designated people about their account. Without this form on file, we are limited in what we are able to discuss with others, such as a spouse or a parent. Only the student may complete the form. Permissions granted by the completion of this form may be rescinded at any time by the student.
